When you join the health mutual, you receive a health-mutual member's pouch. This has been specially designed as an efficient, easy way for you to store all your health-mutual documents and those of dependent persons registered under your membership : yellow stickers (vignettes jaunes), welfare identity card (carte d’identité sociale - SIS)...
The yellow stickers (vignettes jaunes)
The yellow stickers contain all the necessary information to identify you. Stick a yellow sticker in the right place on each document to be sent to the health mutual: certificate of health care dispensed by a doctor, dentist, physiotherapist... Each yellow sticker displays the following information : Each member of the family registered with Mutualité Chrétienne has his/her own yellow stickers. In mind Always stick the relevant yellow sticker on any document to be sent to your health mutual.
The welfare identity card (SIS card)
The SIS card is a welfare identity card. Everyone has one, so each dependent person in your household also has one. The SIS card contains visible and invisible data relating to the different Social Security sectors. The visible data are : The chip contains all the invisible data. Only special equipment can read them. The data are your health mutual's number, your Mutualité chrétienne membership number, the start and end dates of your entitlement to health-care benefits and of your access to the "tiers payant" direct-payment facility. The SIS card contains data other than those intended for the health mutual. You should always keep it on you. You need to produce it to the pharmacist, the hospital or polyclinic, health-care practitioners such as the doctor, dentist, physiotherapist, orthopaedist, home nurse, etc., your delegate, your employer, at the workplace, to the unemployment benefit office, the welfare-insurance office, to the CPAS health-insurance benefit office, etc. For more information...
The persons who are entitled to ask you for your SIS card can read only the data they need to be able to help you.
The SIS card cannot be used abroad. The data for health assistance abroad are on a different card, the "Carte européenne" (European Card) or "World assistance card".
